சுய விவரங்கள் அடங்கிய புகைப்படத்துடன் கூடிய விண்ணப்பத்தில் சான்றொப்பம் அளிக்கப்பட்ட - பத்தாம் வகுப்பு முதல் அனைத்து மதிப்பெண் சான்றிதழ்கள். பட்டயச் சான்றிதழ்கள், சாதிச் சான்றிதழ் மற்றும் வேலைவாய்ப்பு அலுவலகப் பதிவு அட்டை ஆகியவற்றின் நகல்களை இணைத்து செயலர், தேசியக்கல்லூரி (தன்னாட்சி), கருமண்டபம். திருச்சிராப்பள்ளி-620001 என்ற முகவரிக்கு அனுப்பவும். பதவி குறிப்பிடப்பட்ட விண்ணப்பங்கள் மட்டுமே ஏற்றுக்கொள்ளப்படும். ஒவ்வொரு பதவிக்கும் தனித்தனியாக விண்ணப்பங்கள் அனுப்ப வேண்டும்.
